Refund & Cancellation Policy — Primary Care and Wellness Clinic of Arlington

Memberships and monthly plans: Monthly memberships and plans renew automatically each month until canceled. You may cancel at any time by calling (817) 653-4406 or emailing [email protected]. Cancellations take effect at the end of the current billing period, and no further charges will be made after that date.

Refunds: Membership fees cover access to services during the billing period and are non-refundable once that period begins. Partial-month refunds are not issued.

Medications and supplies: Medications, lab work, and supplies are non-refundable once ordered, dispensed, or sent to a pharmacy, as required for patient safety.

Appointments: Please give at least 24 hours' notice to cancel or reschedule. Late cancellations or missed appointments may be subject to a fee.

Billing questions: If you believe you were charged in error, contact us within 30 days at (817) 653-4406 or [email protected]. We will review it and respond within 5 business days.
